I get this missing sub filter icons too sometimes. Most of the time it happens if I open the guild bank or after I have withdrawn some items recently.
It was happening on the current EU live server with version 2.0.15.1194139 (login window version number :D ).

I'm probably not going to remove it until there is another reason to edit the library - it's just a simple if statement.

Okay, well then yeah. ESO does not like loading new addon textures if they were added while the game was already running - that problem is older than ESO 1.0.0

15.2 fixes guild bank filtering in ESO 2.1.x. There are no API changes.
15.2 DOES NOT work on 2.0.x, since the new fragment being hooked does not exist. Should wait until maintenance on Monday to push updates.

As far as I know the problems were solved. It was some old addon he was using if I understood it right.
Should have been fixed after latest update of libFilters.

Second of all - the add-on is very suddenly not working as it used to - the latest update of FCOCraftFilters broke it. I've reverted that and will comment there too, here's the observed behavior:
