It is because the icone is missing from the game. The response from Zenimax for this is: "We didn't put icons for buffs/debuffs for immersion and competitivity". I'm adding them myself 1 by 1.
But it's not just one or two buffs. Every Buff has this error for me. I don't think every Icon is missing :(

There are no Buff Icons, just the Buff/Debuff Background Color
