The Book - List of Names is given when starting the quest Final Blows at Skywatch - at your point south-west of the way shrine. That's the only place a character can acquire it as far as I can see. I has only read a few, of that I am certain. I have reinstalled the add-on (the latest) and allowed the saved variables file to rebuild.
