Great Unitframes so far, two things i would like to see.
1. One thing that no unitframe-addon is paying attention to is the title/npc-flag. I would like to see what kind of merchant i'm dealing with, it's much easier to just see the flag then guess what he's dealing with...
2. Borders would be great - maybe u now TukUi from WoW...

As for the difficulty icons, I will add in a scaling feature, along with the ability to enable or disable them, which I hope would suffice for your request? :)

NPC/Player-title/description on target-frame (below or somewhere else), right now i had to keep the default target-frame on on top for that. Maybe someone has a better idea how to see what kind of merchant i'm dealing with...
