Any addons activated? Test which addon causes it.
Start with inventory related addons adding context menu entries or changing the inventory rows.
I remember there was some addon causing this if you did use the keybind E in the bank without having opened the inventory before the bank was opened (after login/reloadui).
No addons activated -> /bug ingame and tell ZOs
